Role Overview
A clinical RN liaison is needed, being responsible for building and maintaining referral relationships with hospitals, physicians, and healthcare facilities to increase hospice admissions and awareness of services. Combines clinical expertise, education, and relationship-building to support patients, families, and referral partners.
Key Responsibilities
Develop and maintain strong relationships with physicians, hospitals, case managers, and healthcare partners
Conduct regular outreach visits to referral sources to increase hospice awareness and referrals
Promote hospice services and educate providers on care options and eligibility
Support census growth by securing new and maintaining existing referral accounts
Assist in evaluating patient eligibility for hospice services when needed
Meet with patients and families to explain hospice philosophy, services, and obtain consents
Serve as a clinical and educational resource for referral partners
Qualifications
Active, unencumbered RN license
Bachelor's degree in Nursing or related healthcare field preferred
Minimum 1 year of healthcare sales or marketing experience
1–2 years of hospice or palliative care experience strongly preferred
Strong relationship-building and communication skills
Ability to work independently and meet outreach and growth goals
Valid driver's license, reliable transportation, and auto insurance required
