Key Responsibilities
Develop and maintain relationships with physicians, hospitals, discharge planners, case managers, and care facilities
Identify key decision-makers and strengthen referral networks
Promote hospice services and educate healthcare partners on care options and eligibility
Conduct patient and family discussions about hospice philosophy and services
Support patient eligibility assessments when needed
Drive referral growth and maintain census targets through consistent outreach
