Role Overview
As an LMSW at our facility, you will play a critical role in assessing psychosocial risk factors, developing treatment plans, and intervening in high-risk cases. You will be instrumental in advocating for patients and their families, ensuring they receive the support and resources needed for optimal healthcare outcomes.

Key Responsibilities
  • Conduct comprehensive assessments and create tailored treatment plans.
  • Intervene in cases of child abuse/neglect, domestic violence, and complex family situations.
  • Provide guidance on treatment decisions and end-of-life care.
  • Lead discharge planning for complex cases, ensuring continuity of care.
  • Empower patients and families within the healthcare system.
  • Educate on post-acute care options and facilitate informed decision-making.
  • Foster professional growth and collaboration within the healthcare team.

Minimum Qualifications
  • MSW from an accredited program.
  • Current Texas LMSW license.
  • ACM certification preferred.

Preferred Skills/Experience
  • Experience in acute inpatient hospital settings.
  • Proficiency with DSM V and ICD-10.
  • Knowledge of social work assessment, treatment planning, and substance abuse screening.
  • Familiarity with utilization management and payer issues.
  • Excellent communication skills and ability to work as part of a multidisciplinary team.
