We are seeking an experienced and compassionate Lead Care Manager to support Medi Cal members through California’s Enhanced Care Management program.
This is a highly active, community based position for someone who enjoys working directly with individuals who may be experiencing homelessness, chronic health conditions, behavioral health challenges, and other complex barriers to care.
The Lead Care Manager works directly with members in their homes and throughout the community, helping them navigate healthcare and social service systems while coordinating the resources necessary to improve their health, stability, and overall quality of life.
What You’ll Do
Conduct member outreach, interviews, assessments, and eligibility reviews for ECM enrollment.
Develop and maintain individualized care plans based on each member’s needs and goals.
Conduct regular face to face home and community visits with assigned members.
Maintain contact with members through in person visits, telephone, and telehealth.
Coordinate services with healthcare providers, community organizations, and interdisciplinary care partners.
Accompany members to important healthcare appointments when additional support is needed.
Connect members with community resources including housing, food assistance, transportation, financial assistance, healthcare, and other supportive services.
Track referrals and follow up to ensure members successfully receive needed services.
Identify safety, behavioral health, and other urgent concerns and assist with developing appropriate crisis and safety plans.
Complete accurate and timely documentation of member encounters, care coordination activities, and progress notes within the Electronic Health Record system.
Minimum Qualifications
Required
High school diploma or equivalent.
Medical Assistant diploma OR Certified Nursing Assistant diploma is required.
Minimum of 2 years of professional healthcare related experience.
Valid driver’s license.
Clean driving record.
Current automobile insurance.
Reliable personal transportation and willingness to travel extensively throughout the assigned county.
Strong computer skills including Microsoft Office, email, electronic documentation, and virtual communication platforms.
Ability to independently manage a field based caseload and consistently follow through on assignments.
Highly Preferred
Previous experience with Enhanced Care Management, CalAIM, Medi Cal, care coordination, community health, social services, or similar programs.
Experience working with vulnerable or underserved populations.
Knowledge of community resources and social determinants of health.
Bilingual English and Spanish skills are strongly preferred.
This Is a Field Based Position
This position requires approximately 90% travel throughout the assigned county and frequent face to face interaction with members.
Candidates should be comfortable working throughout the community, including member residences, healthcare settings, community organizations, outdoor environments, and other locations where members may need assistance.
The position involves frequent walking, standing, driving, and occasional lifting of supplies weighing up to 20 pounds.
Compensation
$28.00 to $35.00 per hour, based on experience and qualifications.
Mileage reimbursement is provided for approved business travel.
Final compensation will be determined based on relevant experience, professional credentials, specialized skills, and experience within healthcare, social services, Enhanced Care Management, or CalAIM programs.
