POSITION SUMMARY
A California based healthcare and social services organization is seeking an experienced Housing Navigator Specialist to assist Medi Cal members experiencing homelessness or severe housing instability throughout the Los Angeles area.
This position provides intensive housing navigation, client advocacy, landlord engagement, benefits assistance, and care coordination through California’s CalAIM Community Supports program. The Housing Navigator Specialist will help clients secure and maintain safe, stable, permanent housing while addressing barriers such as poor credit, previous evictions, legal concerns, and limited income.
This is a highly active field position requiring frequent travel throughout the Los Angeles service area.
RESPONSIBILITIES
Conduct housing assessments and develop individualized housing stabilization plans.
Help clients identify, secure, and maintain permanent housing.
Assist with housing applications, identity documents, eligibility requirements, and housing program enrollment.
Match clients with appropriate housing opportunities based on their needs and eligibility.
Build and maintain relationships with landlords, property managers, housing authorities, and community organizations.
Advocate for clients and negotiate lease terms when appropriate.
Maintain an organized database of housing resources and available rental units.
Collaborate with Enhanced Care Management teams, healthcare providers, social workers, and community partners.
Participate in case conferences and multidisciplinary care meetings.
Connect clients with mental health, substance use treatment, medical care, and other supportive services.
Assist clients with public benefits such as SSI, General Relief, CalFresh, and other assistance programs.
Educate clients about tenant rights, responsibilities, and housing retention practices.
Provide trauma informed, culturally responsive, and client centered services.
Respond to housing crises, including possible eviction or a return to homelessness.
Maintain accurate and timely documentation within an Electronic Health Record system.
Follow all Medi Cal, CalAIM, HIPAA, and organizational compliance requirements.
QUALIFICATIONS
Bachelor’s degree in Social Work, Human Services, Public Health, or a related field required. Equivalent direct field experience may be considered.
At least two years of professional experience working directly with individuals experiencing homelessness or other high risk populations.
Experience with CalAIM, Medi Cal, Community Supports, or Enhanced Care Management is strongly preferred.
Knowledge of Los Angeles housing resources, HUD, Section 8, local housing authorities, affordable housing programs, and community services.
Understanding of trauma informed care and harm reduction principles.
Strong communication, negotiation, advocacy, problem solving, and time management skills.
Ability to build trust and rapport with individuals from diverse backgrounds.
Ability to work independently and collaborate with multidisciplinary teams.
Bilingual fluency in Spanish is preferred.
Ability to travel frequently throughout the Los Angeles area using a personal vehicle.
Ability to work in community environments, including shelters, encampments, and outdoor locations.
Ability to occasionally lift and carry up to 20 pounds.
SCHEDULE
Monday through Friday from 8: 00 a.M. To 5: 00 p.M. Occasional evening and weekend availability may be required based on client needs.
