Position Overview: We are seeking a skilled and compassionate Physical Therapist (PT) to join our long-term care facility team. As a PT, you will play a pivotal role in improving the quality of life for our residents by providing effective rehabilitation services. Your expertise in assessing, planning, and implementing therapeutic interventions will contribute to restoring and enhancing residents' mobility, strength, and functional independence.
Responsibilities:
Assessment and Evaluation:
Conduct thorough evaluations of residents' physical conditions, mobility, and functional abilities.
Identify areas of concern and develop personalized treatment plans to address residents' specific needs and goals.
Treatment Planning and Implementation:
Create comprehensive treatment plans that include exercises, manual techniques, and modalities to improve residents' physical function and alleviate pain.
Administer therapeutic interventions and monitor progress, adjusting plans as needed for optimal outcomes.
Patient Education:
Educate residents and their families about treatment goals, exercises, and strategies for maintaining and improving functional abilities.
Provide guidance on proper body mechanics and assistive devices to enhance residents' safety and mobility.
Collaboration:
Collaborate with interdisciplinary teams, including physicians, occupational therapists, speech therapists, and nursing staff, to ensure integrated care plans.
Communicate regularly with team members to share progress updates and discuss adjustments to treatment plans.
Documentation:
Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and other relevant information by facility policies.
Safety and Compliance:
Adhere to infection control protocols and facility safety standards while delivering care.
Ensure compliance with state and federal regulations and ethical guidelines for physical therapy practice.
Quality Improvement:
Participate in quality improvement initiatives to enhance the overall quality of care provided to residents.
Contribute to ongoing professional development by staying informed about new research and best practices in physical therapy.
Qualifications:
Current and valid Physical Therapist (PT) license in the state.
Minimum of [X] years of experience as a practicing Physical Therapist.
