Role Overview: As a dedicated Case Manager, you will play a pivotal role in enhancing patient care coordination and clinical outcomes. Key Responsibilities:
Interdisciplinary Collaboration: Work closely with physicians and medical teams to streamline patient care.
Problem-Solving: Address and resolve systemic issues affecting patient progress.
Discharge Planning: Efficiently manage the discharge process, ensuring timely transitions.
Care Monitoring: Oversee patient progress and adapt care plans as needed.
Utilization Management: Assign care levels and comply with quality screening standards.
Continuous Improvement: Enhance length of stay and resource utilization metrics.
Professional Development: Engage in activities that contribute to team growth and improvement.
Qualifications:
Graduate of an accredited school of professional nursing;BSN preferred OR
Graduate of an accredited Master of Social Work program.
Valid Texas RN or
LMSW license;LCSW preferred.
Case Management (ACMA) certification (required within two years of hire).
3+ years of acute care case management experience.
Strong data management, communication, and leadership capabilities.
